When you first contact ACM Care, we will discuss with you whether our services are a good fit for you or if a simpler, more cost-effective solution is available. There is no charge for this free initial phone assessment.
Once you decide our services are a good match, we will quote you a price based upon the ACM Care services you select and will schedule an initial onsite appointment with the client and any family who choose to be present.
During our initial appointment, your ACM Care Manager will review with you and obtain signatures consenting to the selected ACM Care services, the confidentiality standards of ACM Care, and HIPAA release forms. We also answer any initial questions you may have and discuss the challenges at hand in depth to further understand the existing situation from all parties' point-of-view. This discussion also allows us to take stock of all of the personalities involved to better customize our recommendations and/or services.
From this original onsite assessment, we will finalize your individualized ACM Care action plan. Your plan will outline the exact services that will be provided, how often these services will be provided as well as address any concerns you may have as we work with you to improve the current situation. In addition to all of this, if we do not currently provide a particular service necessary to meet a specific need you have, we will help you find someone who will.
Once we begin on the agreed upon journey, sometimes it may not appear evident that progress is being made quickly. ACM Care takes great care to build a trusting relationship and unravel the steps that led up to the current situation in order to successfully navigate the journey before us.
