The ACM Care goal is to improve the quality of life for elderly and disabled people. We strive to achieve this by helping individuals remain in or return to their home and be successful there. In times where we have to help someone choose a nursing home or improve the care they are receiving within a nursing home, our mission is always to help the client achieve their goals while helping them to feel successful and cared for wherever they may be.
Your first step is to call ACM Care today. We offer everyone a free assessment (by phone) with one of our professional case managers to determine if our services are a good match to improve your situation or if a simple answer to a question is all you need. We may ask you to provide simple demographic information for tracking purposes and quality control, but the initial call is a complementary service we provide.
We know that most of us, and our parents, do not want to admit that we are losing control of a situation. It can be a scary and upsetting time for all of those involved. The best way to introduce our services to your parents is to be honest about your goals and how ACM Care, through our commitment to preserving their rights and their dignity, is dedicated to helping people remain safe and healthy in their home (or the least-restrictive environment) for as long as possible. With the support of ACM Care, families can return to being families and parents can once again enjoy their family time without the concern of being consumed with day-to-day tasks or correcting problems. If you have a unique situation, we encourage you to discuss it with one of our case managers before your initial meeting.
ACM Care is very different than most traditional case management providers as we have earned a quality reputation of being very solution-focused and successful at problem-solving when traditional answers no longer work. While each client has an assigned personal Case Manager, our team approach and the team's collective expertise provides a broader array of knowledge, experience, and personalities to best address the unique needs of each one of our clients. All of the services provided by ACM Care are tailored to fit your needs as well as your budget. We work hard for you when you want, we gently guide you through what you want to manage yourself, and we support you during those difficult periods of uncertainty.
Nursing homes are required, by law, to protect their residents and their resident's privacy. We suggest that once you decide to hire us, you contact the nursing home administrator to inform them that ACM Care will be providing case management services and will need access to the nursing home chart. Most nursing homes will have their own HIPAA consent form that they require to be signed by the responsible party and possibly the resident, which gives the nursing home permission to let us review all medical records. If at all possible, we suggest this be accomplished before the first onsite meeting, since without consents signed, the nursing home may prevent us from gathering all the information we need to get started. If the nursing home has computerized files, there may be additional paperwork to sign, granting us access to the resident's specific computerized record.
ACM Care makes every effort to keep your personal information confidential, just as any medical office or health care establishment. Paper records are kept within locked cabinets, computer files are password protected, and only the minimal required information is shared with outside parties. We understand that over time family "skeletons" may be revealed or your loved one may divulge confidential information or feelings to us that they do not want shared. Legally, we are bound by law to report any incidences of abuse, neglect, and exploitation to the authorities. Ethically, we make every effort to only release absolutely necessary information during the case management relationship so that our client gets the care and services they need, without others knowing "their business." This is extremely important as the foundation of our success with our clients is built on the trust they place with us.
Yes, you get your own case manager who will complete your initial onsite visit, the necessary paperwork, and coordinate all your services and communication. During your initial phone call, ACM Care will recommend a specific staff person to you, based on your needs and your choices identified within our initial phone assessment. If your case manager identifies an issue that is within an area of expertise of a different case manager, then your case manager will consult with our in-house expert at no charge to you. If your needs change and would be best met by a different case manager or even a different program through ACM Care, we will discuss it with you beforehand allowing you to make the ultimate decision.
