ACM Care (Autonomous Case Management of St Louis Inc) began in 1999 as a not-for-profit organization after co-founder and Registered Nurse, Julie Hess, frequently witnessed elderly patients in hospitals without the support of family and thus advocacy for their healthcare. These same individuals, alone and without sufficient resources, were often told by medical professionals that they had no choice but to be admitted to a nursing home. Hess knew other options existed and decided it was time to help.
ACM Care was modeled after a Connecticut case management organization that has been in practice for over 25 years. Their model was based on a team approach between nurses and social workers to provide tailored solutions to clientele who neither desired nor needed to make the move into a nursing home. The agency was funded from three sources: private fee-for-service contracts with individuals, government contracts for cost-saving projects, and donations. Since its inception in 1999, ACM Care has continued to grow and expand the services provided to better meet the challenges of our clients.
